Instructions
To earn credit online, you must listen to the CME lectures carefully, print out the quiz page and answer both the quiz and lecture evaluation questions, obtain a passing score (a minimum of 75% when 4 questions are present), and complete the credit evaluation form. Your certificate will be emailed to you at the email address provided within 3 weeks. Please mail or fax your completed quiz to the following address:

Accreditation information
This CME activity offers the opportunity to assess your knowledge and retention of the information presented in the Grand Rounds lecture and to earn continuing medical education credit.

This activity has been planned and implemented in accordance with the Essential Areas and policies of the Accreditation Council for Continuing Medical Education through the joint Accreditation Council for Continuing Medical Education through the joint sponsorship of Interstate Postgraduate Medical Association (IPMA) and McGraw-Hill Medical Publishing. IPMA is accredited by the ACCME to provide continuing medical education for physicians.

IPMA designates this education activity for a maximum of 01 Category 1 credits toward the Physician's Recognition Award of the American Medical Association. Each physician should claim only those credits he or she actually spent in the activity.
